Misuse of over-the-counter pain medication is potential health threat

ScienceDaily (May 30, 2012) ? A significant number of adults are at risk of unintentionally overdosing on over-the-counter (OTC) pain medication, according to a new study in the US by Dr. Michael Wolf, from Northwestern University in Chicago, and his colleagues. Their work, looking at the prevalence and potential misuse of pain medication containing the active ingredient acetaminophen as well as the likelihood of overdosing, appears online in the Journal of General Internal Medicine, published by Springer.

Many adults in the US regularly use OTC pain medication containing the active ingredient acetaminophen, the most commonly used OTC pain medication in the US. They take it either on its own or in combination with other drugs, which may also contain acetaminophen. The ease of access to OTC drugs presents a challenge to patient safety as many individuals may lack the necessary health literacy skills to self-administer these medicines appropriately. Indeed, individuals make independent decisions that match an OTC product to a self-diagnosed symptom or condition. Worryingly, acetaminophen overdose is the leading cause of acute liver failure.

Wolf and colleagues interviewed 500 adult patients receiving care at outpatient general medicine clinics in Atlanta and Chicago between September 2009 and March 2011. Over half the patients reported some acetaminophen use and 19 percent were 'heavy users' i.e. they had taken it every day, or at least a couple of times a week, during the previous six months.

The researchers tested whether these patients understood the recommended dosage and were able to self-administer OTC acetaminophen appropriately. Firstly, could they work out the proper dosing of a single OTC medication over a 24-hour period? Secondly, what was the risk of patients 'double-dipping', or simultaneously taking two acetaminophen-containing products, and thereby exceeding the recommended dose?

To assess proper dosing, the participants were given five OTC drug bottles and, for each one, were asked to imagine that they took the first dose at time X, and wanted to take the maximum dose of this medicine in one day. They were then asked to show the researcher how many pills and at which times they would need to take them for a full 24-hour day.

To assess 'double-dipping', the patients were told to imagine they were taking a maximum dose of a primary OTC drug and asked whether it would be safe to also take a second medicine with the primary medicine -- both of which contained acetaminophen.

Wolf and team found that nearly a quarter of the participants were at risk of overdosing on pain medication using a single OTC acetaminophen product, by exceeding the dose of four grams in a 24-hour period; 5 percent made serious errors by dosing out more than six grams. In addition, nearly half were at risk of overdosing by 'double-dipping' with two acetaminophen containing products.

The authors conclude: "Our findings suggest that many consumers do not recognize or differentiate the active ingredient in OTC pain medicines, nor do they necessarily closely adhere to package or label instructions. Given the prevalence of the problem, risk of significant adverse effects, and lack of a learned intermediary i.e. a physician to guide decision making and counsel consumers on proper use, we believe this to be a serious public health threat requiring urgent attention."

Parent?child communication is another dimension or process in family relationships, and its association with adolescent pregnancy risk has been investigated in more than 30 studies . Results across these studies are so variable and discrepant that no simple, direct effect is discernible. There is little or no agreement between what parents and teens perceive to have been communicated between them , and there is no consistency in findings about whether parents? or teens? reports of their communication are related to adolescent pregnancy risk. In most studies that have tested these relationships by parent and child gender, mothers? communication is more likely to be associated with adolescent pregnancy risk variables than father?s communication, and there is a stronger effect for daughters than for sons. Several important conceptual and methodological issues complicate our understanding of the association between parent?child communication and adolescent pregnancy risk . One is the temporal ordering of variables; there is a theoretical basis for prior parent?child sexual communication to predict more responsible adolescent sexual behavior, especially if parents disapprove of their child?s having sex and there is a close parent?child relationship. On the other hand, if teen sexual behaviors are known to, or suspected by parents, they might begin or intensify their communication with teens about sex and contraception. Adolescent Pregnancy 573 Further, associations between parent/teen communication and adolescent sexual behavior are moderated by parents? values. Researchers have demonstrated that parents? sexual values, in combination with parent?child communication, have an important effect on adolescents? intercourse experience. Mediating mechanisms have been identified that could help explain how parent/ child relationships influence adolescents? sexual behavior. For example, in one study the effects of parental warmth and involvement in the seventh grade were shown to affect teen pregnancy status in 12th grade through intervening mechanisms such as deviant peer affiliations, substance use, delinquency, and academic competence. Results of other studies indicate that parent?child closeness is related to mediating mechanisms such as teens? attitudes about having intercourse, teens? depression, impulse control, academic and prosocial activities, and teens? use of substances and association with sexually active peers, all of which are related to adolescent sexual behavior and pregnancy. In particular, several investigators have suggested that a lack of closeness in the parent?teen relationship increases the negative influence of peers on adolescent sexual activity. On the positive side, parent?child closeness and involvement can reduce teen sexual behavior by enhancing educational achievement, providing youth with opportunities to develop prosocial skills, and helping teens acquire a sense of competence and worth .

Women, keep those toothbrushes and dental floss handy. A comprehensive review of women's health studies by Charlene Krejci, associate clinical professor at the Case Western Reserve University School of Dental Medicine, has shown a link between women's health issues and gum disease.

Across the ages, hormonal changes take place during puberty, menstruation, pregnancy and menopause. Krejci found female hormones that fluctuate throughout women's lives can change conditions in the mouth that allow bacteria to grow, enter the blood, and exacerbate certain health issues like bone loss, fetal death and pre-term births.

Her overview of the literature was reported in the article, "Women's Health: Periodontitis and its Relation to Hormonal Changes, Adverse Pregnancy Outcomes and Osteoporosis" in the May issue of Oral Health and Preventive Dentistry.

The Case Western Reserve University periodontist reviewed 61 journal articles with nearly 100 studies for a collective answer on whether hormones have a relationship to gum disease and specific women's health issues like preterm labor, bone loss, and the side effect of hormonal replacement therapy.

"There's definitely a gender-specific connection between women's hormones, gum disease, and specific health issues impacting women," Krejci said.

"Although women tend to take better care of their oral health than men, the main message is women need to be even more vigilant about maintaining healthy teeth and gums to prevent or lessen the severity of some of women-specific health issues," Krejci said.

In addition to the brushing and flossing daily regimen, Krejci recommends visiting the dentist at least every six months, and more if there are any gum problems found or women suffer from bone loss or are pregnant.

She added that it is widely known that hormones cause some women gum problems during pregnancy. Women already susceptible to gum disease before being pregnant, she advises, need to make sure that these oral problems are treated.

Although women were once discouraged from seeing the dentist while pregnant, she said that scaling and planing of the roots of teeth to eliminate some gum disease is now recommended during pregnancy for women. Severe gum disease requiring surgery is still generally postponed until after the baby's birth.

Gum disease begins with the build up of bacterial plaque on the teeth and under the gums. Untreated it can cause irritation and inflammation during which harmful and toxic byproducts are released. These toxins erode the bone that anchors teeth and cause breaks and bleeding in the gums.

Sally had always dreaded the thought of having to relocate again. The last few years she had moved several times because of her work. It was beginning to become a burden each time she had to pack her life up and start over. The only saving grace in her move was she was using moving pods. That made her move much easier.

Moving was always difficult, but trying to find a new home was just as hard. Generally what she would do at night would be surfing the Internet looking at all of the different options she had as far as new places to live. Right before her move she found a place that seemed perfect. Not only was it cozy but the landlord agreed to see her immediately. Once she arrived to her destination she really liked what she saw. The place seemed perfect for her, and for once she thought she would stay for a while. The landlord was friendly and told her he could have the place available immediately so she would not have to wait.

The night before was always difficult for Sally. Right before she left one house and got ready to move into another she would often reflect about all of the decisions she had made over the years. She was now in her 30s and realized that she had given up everything for her career. She never had a family, and had always made her job number one. That sometimes bothered her, but she always kept pushing forward with her job.

Moving day rapidly approached. The good news is that she was ready to go and so was the moving company. Everything went well, so she was very happy about that.

Driving to her new place felt different this time. She felt like she was going to stay longer than normal. She was hoping this new place would feel like a home to her.

Her landlord could sense that she was very eager to move in. He had her sign a few documents and gave her the keys and was on his way. Sally appreciated that, and sat on her living room floor looking at her new place.

She heard a large truck pull up and realized all of her moving pods were here. A great sense of relief came over her because she knew it was up to her to get her new place made into a home.

Attempts to revive language spoken in Jesus' time

JISH, Israel (AP) ? Two villages in the Holy Land's tiny Christian community are teaching Aramaic in an ambitious effort to revive the language that Jesus spoke, centuries after it all but disappeared from the Middle East.

The new focus on the region's dominant language 2,000 years ago comes with a little help from modern technology: an Aramaic-speaking television channel from Sweden, of all places, where a vibrant immigrant community has kept the ancient tongue alive.

In the Palestinian village of Beit Jala, an older generation of Aramaic speakers is trying to share the language with their grandchildren. Beit Jala lies next to Bethlehem, where the New Testament says Jesus was born.

And in the Arab-Israeli village of Jish, nestled in the Galilean hills where Jesus lived and preached, elementary school children are now being instructed in Aramaic. The children belong mostly to the Maronite Christian community. Maronites still chant their liturgy in Aramaic but few understand the prayers.

"We want to speak the language that Jesus spoke," said Carla Hadad, a 10-year-old Jish girl who frequently waved her arms to answer questions in Aramaic from school teacher Mona Issa during a recent lesson.

"We used to speak it a long time ago," she added, referring to her ancestors.

During the lesson, a dozen children lisped out a Christian prayer in Aramaic. They learned the words for "elephant," ''how are you?" and "mountain." Some children carefully drew sharp-angled Aramaic letters. Others fiddled with their pencil cases, which sported images of popular soccer teams.

The dialect taught in Jish and Beit Jala is "Syriac," which was spoken by their Christian forefathers and resembles the Galilean dialect that Jesus would have used, according to Steven Fassberg, an Aramaic expert at the Hebrew University in Jerusalem.

"They probably would have understood each other," Fassberg said.

In Jish, about 80 children in grades one through five study Aramaic as a voluntary subject for two hours a week. Israel's education ministry provided funds to add classes until the eighth grade, said principal Reem Khatieb-Zuabi.

Several Jish residents lobbied for Aramaic studies several years ago, said Khatieb-Zuabi, but the idea faced resistance: Jish's Muslims worried it was a covert attempt to entice their children to Christianity. Some Christians objected, saying the emphasis on their ancestral language was being used to strip them of their Arab identity. The issue is sensitive to many Arab Muslims and Christians in Israel, who prefer to be identified by their ethnicity, not their faith.

Ultimately, Khatieb-Zuabi, a secular Muslim from an outside village, overruled them.

"This is our collective heritage and culture. We should celebrate and study it," the principal said. And so the Jish Elementary School become the only Israeli public school teaching Aramaic, according to the education ministry.

Their efforts are mirrored in Beit Jala's Mar Afram school run by the Syrian Orthodox church and located just a few miles (kilometers) from Bethlehem's Manger Square.

There, priests have taught the language to their 320 students for the past five years.

Some 360 families in the area descend from Aramaic-speaking refugees who in the 1920s fled the Tur Abdin region of what is now Turkey.

Priest Butros Nimeh said elders still speak the language but that it vanished among younger generations. Nimeh said they hoped teaching the language would help the children appreciate their roots.

Although both the Syrian Orthodox and Maronite church worship in Aramaic, they are distinctly different sects.

The Maronites are the dominant Christian church in neighboring Lebanon but make up only a few thousand of the Holy Land's 210,000 Christians. Likewise, Syrian Orthodox Christians number no more than 2,000 in the Holy Land, said Nimeh. Overall, some 150,000 Christians live in Israel and another 60,000 live in the West Bank.

Both schools found inspiration and assistance in an unlikely place: Sweden. There, Aramaic-speaking communities who descended from the Middle East have sought to keep their language alive.

They publish a newspaper, "Bahro Suryoyo," pamphlets and children's books, including "The Little Prince," and maintain a satellite television station, "Soryoyosat," said Arzu Alan, chairwoman of the Syriac Aramaic Federation of Sweden.

There's also an Aramaic soccer team, "Syrianska FC" in the Swedish top division from the town of Sodertalje. Officials estimate the Aramaic-speaking population at anywhere from 30,000 to 80,000 people.

For many Maronites and Syrian Orthodox Christians in the Holy Land, the television station, in particular, was the first time they heard the language outside church in decades. Hearing it in a modern context inspired them to try revive the language among their communities.

"When you hear (the language), you can speak it," said Issa, the teacher.

Aramaic dialects were the region's vernacular from 2,500 years ago until the sixth century, when Arabic, the language of conquering Muslims from the Arabian Peninsula, became dominant, according to Fassberg.

Linguistic islands survived: Maronites clung to Aramaic liturgy and so did the Syrian Orthodox church. Kurdish Jews on the river island of Zakho spoke an Aramaic dialect called "Targum" until fleeing to Israel in the 1950s. Three Christian villages in Syria still speak an Aramaic dialect, Fassberg said.

With few opportunities to practice the ancient tongue, teachers in Jish have tempered expectations. They hope they can at least revive an understanding of the language.

The steep challenges are seen in the Jish school, where the fourth-grade Aramaic class has just a dozen students. The number used to be twice that until they introduced an art class during the same time slot ? and lost half their students.

Astronauts enter world's 1st private supply ship

CAPE CANAVERAL, Fla. (AP) ? The space station astronauts have entered the Dragon.

Early Saturday, the crew of the International Space Station slid open the hatch of its new addition, the world's first commercial supply ship. The SpaceX capsule, named Dragon, made history with its arrival Friday.

NASA astronaut Donald Pettit was the first one in. He said it smelled like a brand new car and reminded him of the cargo capability of his pickup truck. Pettit and his crewmates have until the middle of next week to unload Dragon's groceries and refill the capsule with gear for return to Earth.

The California-based SpaceX is the first private company to send a vessel to the space station. NASA is turning over delivery work so it can focus on getting astronauts to asteroids and Mars.

I bought a crock pot awhile ago, thinking that I found the solution to my nightly dinner dilemma. Like so many other ?crock pot fanatics?, the bloom was off the rose as soon as I found myself with a bit more time and money to spend. So, my crock pot found it?s way into the deep, dark recesses of my pantry, never to be heard from again. Or so I thought. My budget is tighter than ever and time is at a premium, as well. Dinnertime is again centered on a warm and wonderful crock pot meal, much to the delight of my family.

Now, if you bought your crock pot more than, say, 6 years ago, look into getting a new one. They have improved so much that it?s worth the $35 or $40 you?ll be spending. The newer crock pots have better energy efficiency, plus features like warming cycles, latched tops, temperature settings, and all kinds of fun stuff. But one of the best features about using a crock pot is that they save you money and time. Seems like a good thing, but how do they figure that?

Money Saving: Certain dishes, like beef stew, pot roasts, and soups, are usually cooked for a pretty long period of time. If you?re using your oven for, say 3 hours, that?s about 10 kWh for that meal. Cooking the same pot roast in a crock pot for, say 6 hours, will take about 1.2 kWh. That?s simply based on your oven wattage of roughly 2500 versus your crock pot wattage of roughly 200. It just makes sense using a smaller appliance for such a long cooking period. Check out your appliance wattage and energy consumption using any of the energy consumption calculators online and see if you don?t agree.

Most crock pot recipes call for inexpensive cuts of meat. The low, slow cooking of a crock pot breaks down the sinew and connective tissue of less expensive cuts of meat. Crock pot recipes call for cooking in liquid which also tenderizes tougher cuts of meat, so you end up with meat that falls apart with a fork. A more tender cut doesn?t stand up as well to the long, slow cooking or the liquid. Opt for beef cuts like brisket, bottom round, rump roast, round steak, or even a good chuck roast. Pork shoulders and short ribs are good, too.

Are you spending money on fast food? How many times have you rushed through the convenience store, spending money you didn?t have on bad food? Here?s where your crock pot can save you money with a little planning. Get your meal planned, shop, and prepare everything ahead of time. Now, when everybody gets home after school and work, and all that evening chaos begins, at least you know you have dinner bubbling away in the crock pot. No more quick, and expensive, stops for fast food.

When you are air conditioning your house, the last thing you want to do is heat up your kitchen by having the oven on for a long time. During the summer, we do eat mostly cold meals, like salads, but summers can be pretty long here and my family starts to get a yearning for a nice, hot meal. Without adding a bunch of hot air to my house, I can throw a pot roast in my crock pot, and we?re all happy? and the air conditioner can do it?s normal work without stressing it out anymore than necessary.

Time Saving: If you are watching your pots boil, you are wasting time. Even stirring soups and sauces take time, especially if you?re making something that could stick to the bottom of the pot if left unattended. When you make a meal in one pot it truly does save time, but not if you have to watch it! Sure, you can put a pot roast in the oven and you won?t have to stand over it while it cooks, but now you?re not saving money. Don?t we want to save both money and time?

Getting my family to help with meal preparations is a big time saver in my book. If I can walk away for a few minutes while my children are scrubbing potatoes and plunking them in the crock pot, I?ve got those precious minutes to attend to another task. Getting kids involved putting a crock pot meal together is possible because there?s less danger of having someone get burned; you don?t turn on a crock pot until all the ingredients are in and you?re ready. There are many crock pot recipes available that have only a few basic ingredients, so even a cooking novice like my husband can jump right in there and cook a meal, too.

If you?ve been running to the grocery store several times a week to hurry up and get something for dinner, you?ve been wasting a lot of your very precious time. Wouldn?t it be nice to pick up the kids from swim lessons and be able to go right home without that frantic stop for food? I keep several of my favorite crock pot recipes in my purse, so all I have to do is make one planned stop at the grocery store, buy the ingredients I need for a couple meals, and my shopping is done for the week. I even have time now to clean all my vegetables when I get home so they?re all ready when I put my planned crock pot dinners together. Now, we?re really saving time!

Money and Time Saving: I?m a big believer in cooking larger portions so that we?re cooking once and eating twice. My crock pot is a 6 quart size oval, which is big enough to have leftovers for lunches, so we don?t have to spend money buying lunches at work or school. We also will cook double and put half in the freezer. This way, I?m cooking only two or three times a week, and we?re eating from the freezer the other days. For us, that?s a real time and money saver.

Josh Trank Stands in the Shadow of the Colossus

Despite the unraveling of what was going to be a very strange (read: unwanted) adaptation of the Uncharted?video games, Sony Pictures is continuing to push the genre forward through developing several other brands for the live-action treatment. Sony signed with Ubisoft Motion Pictures to develop the?Assassin?s Creed?film in a deal that didn?t sit well with rival studios and as we learned today, they?re still planning a live-action feature film based on Team Ico?s?Shadow of the Colossus.

Deadline?has the update on the latest important video game title that?s entering development, a place where countless projects still sit as the video game movie genre remains in an uneasy place, unproven but ripe for?emergence. Sony aims to make Shadow of the Colossus?a large-scale blockbuster with the help of producer?Kevin Misher (Mirror Mirror) and have signed Josh Trank (Chronicle) to direct. Back in 2009, Sony had hired?Justin Marks (Street Fighter: The Legend of Chun-Li) to pen the screenplay but that evidently did not work out.

Trank hit it big with his low-budget found footage film Chronicle?earlier this year and has since become widely sought over for several big budget blockbusters, including the Fantastic Four?reboot and Sony?s own Venom?- a spinoff for The Amazing Spider-Man. With Shadow of the Colossus?however, Trank may have found his biggest challenge yet.

The story of the game isn?t its focus, nor is its characters. In fact, there are not many characters at all in the game which barely features any dialogue. It?s about a boy?named Wander who travels with his trusty companion and only ally ? a horse named Agro ? across vast open areas, hunting a series of 16 massive and unique Colossi with his stolen magic sword in order to revive Mono, a young girl and his love interest.

The game earned critical praise for its uniqueness in style, its impressive visuals and its award-winning soundtrack, and that?s partially why a film adaptation is difficult to fathom. Unlike most video game properties, Shadow of the Colossus?isn?t a property that can spawn a continuing franchise.?The game didn?t feature much dialogue and there weren?t populated areas with other characters -?It was a visual spectacle and more of a puzzle game than a typical action-adventure.

This begs the question of how faithful a Sony Pictures film adaptation could really be. Can they sell a movie to mainstream moviegoing audiences about one kid fighting gigantic monsters, entirely in CGI? Will they force in other characters and backstory a la Prince of Persia?and take away from what made the game unique? Since it is so dependent on visuals, will it look more real than Wrath of the Titans?

While our Game Rant team loves Shadow of the Colossus?it?s hard to imagine at this point how it will be adapted for the silver screen, especially considering its heavy budgetary requirements for the effects. We can all expect this one to be made for 3D but we do not know what to expect from the script and story. Sony is looking at writers to join Trank at the moment and we?re intrigued with the idea of a live-action take.

BAKU, Azerbaijan (AP) ? A memorable melody. Distinctive stage presence. Some horse-trading of votes.

Those are secrets to success in the annual Eurovision Song Contest, a televised pan-European extravaganza viewed by some 125 million people worldwide that is now entering its 57th year.

The winner is picked by juries and television viewers across the continent. Semifinals this week have whittled down the entries to 26. A smorgasbord of revealing outfits and onstage preening is expected at Saturday's final, but gray-haired acts from the U.K and Russia are stealing most of the attention.

Here's a look at favorites and wacky wonders:

ENGELBERT HUMPERDINCK

Engelbert Humperdinck of the U.K. had turned 20 and was already a seasoned pop performer by the time the first Eurovision Song Contest was held ? in 1956. In the 1960s, his manager convinced him to change his stage name from Gerry Dorsey to Engelbert Humperdinck? after a 19th century German composer ? and he went on to become a less raunchy version of Tom Jones. Amid all of Eurovision's hyper-kinetic dance and pop acts, the 76-year-old's "Love Will Set You Free" stands out as good old-fashioned crooning.

BURANOVSKIYE BABUSHKI

The gray hairs are taking over Eurovision. The (old) girls from Buranovskiye Babushki from the Russian Urals are doing their bit to confirm all those stereotypes about shawl-wearing grannies. They are almost certainly the first Eurovision contestants to perform part of their song in the obscure Udmurt language, which is distantly related to Finnish. Their sheer adorableness gives them universal appeal and their folky, up-tempo "Party for Everybody" is hard to dislike completely.

JEDWARD

The irrepressibly, and often annoyingly, enthusiastic Jedward twins from Ireland first came to prominence in 2009 on the British television talent show "The X Factor." They failed to win but have since fashioned a faintly successful pop career. They are competing for the second year running, possibly because the last thing Ireland wants now is to win and have to host the expensive contest next year. Jedward's song "Waterline" is pretty odious fare, both tinny and thumping at the same time.

LOREEN

Sweden's Loreen is the bookmakers' runaway favorite. The 28-year-old of Moroccan-Berber descent easily swept aside all rivals in Sweden's competition to pick its entrant for Eurovision. Her song "Euphoria" even stands a chance of fair international commercial success ? it's already topped the Swedish charts for six weeks and gone platinum five times.

MANDINGA

Romania's entry, a band fronted by sultry vocalist Elena Ionescu, is performing its richly overdone attempt at exotica in both Spanish and English, possibly seeking the Iberian vote. It's song "Zaleilah" is a global mishmash: Cuban horns, lashings of salsa, a generous dollop of Gypsy frenzy, and even a smattering of bagpipes. An earphone malfunction hurt rehearsals but bookmakers still expect a strong showing.

ZELJKO JOKSIMOVIC

At the staid end of the spectrum, Serbia's Zeljko Joksimovic is playing it straight with his slow and stripped-down "Nije Ljubav Stvar." He is a Eurovision veteran, having come second in 2004 and penned Serbia's entries in 2006 and 2008. He has struck an international note by releasing versions of his song in English, Spanish and Russian ? and even produced an Azeri version this week at a party in Baku. His chances aren't bad ? countries from the former Yugoslavia often perform well due to their habit of voting for one another.

NINA ZILLI

"Out of Love" is a brassy, lively ode by Italy's Nina Zilli that could spring a surprise. Italy has passed on the competition most years, citing a lack of interest, but Zilli's confident performance could turn that around. The first part of her stage name is a tribute to jazz artist Nina Simone, and if you listen really really hard, you can hear those hints of soul.
