Higher death risk with sleeping pills

ScienceDaily (Feb. 27, 2012) ? People are relying on sleeping pills more than ever to get a good night's rest, but a new study by Scripps Clinic researchers links the medications to a 4.6 times higher risk of death and a significant increase in cancer cases among regular pill users.

The results, published?February 27 by the open-access online journal BMJ Open, cast a shadow over a growing segment of the pharmaceutical industry that expanded by 23 percent in the United States from 2006 to 2010 and generated about $2 billion in annual sales.

The possible health hazards

"What our study shows is that sleeping pills are hazardous to your health and might cause death by contributing to the occurrence of cancer, heart disease and other ailments," said author Daniel F. Kripke, MD, of the Viterbi Family Sleep Center at Scripps Health in San Diego.

The research is the first to show that eight of the most commonly used hypnotic drugs were associated with increased hazards of mortality and cancer, including the popularly prescribed medications zolpidem (known by the brand name Ambien) and temazepam (also known as Restoril), Dr. Kripke said. Those drugs had been thought to be safer than older hypnotics because of their shorter duration of action.

Study participants who took sleeping pills were matched with control patients of similar ages, gender and health who received no hypnotics in order to eliminate the possibility that other factors led to the results.

"We tried every practical strategy to make these associations go away, thinking that they could be due to use by people with more health problems, but no matter what we did the associations with higher mortality held," said co-author Robert D. Langer, MD, MPH, of the Jackson Hole Center for Preventive Medicine in Jackson, Wyoming.

Even among patients who were prescribed 1 to 18 sleeping pills per year, the risk of death was 3.6 times higher than among similar participants who did not take the medica-tions. The study looked at patients aged 18 years and older, and found the increased risk in all age groups.

Sleeping pills and cancer

Rates of new cancers were 35 percent higher among patients who were prescribed at least 132 hypnotic doses a year as compared with those who did not take the drugs.

Using data stored in an electronic medical record that has been in place for more than a decade, the researchers obtained information on almost 40,000 patients cared for by a large integrated health system in the northeastern United States.

The study included 10,531 sleeping pill users who were prescribed the medications for an average of 2.5 years and 23,674 control participants who were not prescribed the drugs. Information came from outpatient clinic visits conducted between Jan. 1, 2002, and Sept. 30, 2006.

"It is important to note that our results are based on observational data, so even though we did everything we could to ensure their validity, it's still possible that other factors explain the associations," said co-author Lawrence E. Kline, DO, who is medical director of the Viterbi Family Sleep Center. "We hope our work will spur additional research in this area using information from other populations."

Funding for the study came from the Scripps Health Foundation and other philanthropic sources.

Alternatives to medication

The BMJ Open report should prompt physicians to consider alternatives to hypnotic medications, Dr. Kline said.

Clinicians at the Viterbi Family Sleep Center focus on cognitive therapy that teaches patients to better understand the nature of sleep. For example, some people suffering from insomnia might require less than the eight hours of sleep commonly recommended for each night.

Patients also can benefit from practicing good sleeping habits and relaxation, as well as taking advantage of the body's natural clock, which is driven by the rising and setting of the sun, Dr. Kline said. "Understanding how to use the circadian rhythm is a very powerful tool that doesn't require a prescription," he said.

When insomnia results from emotional problems such as depression, doctors should treat the psychological disorder rather than prescribe sleeping pills that could prove to be harmful, Dr. Kripke said.

General comments

The submitted article provides an account of the difficult times education in Tanzania is going through. In particular, it describes the decline in academic performance, with and emphasis on secondary school students. Although this is a topic which is recurrently addressed in the literature, it is always of interest given the importance for all countries of improving the opportunities to access to education, as well as improving qualitatively and quantitatively the educational quality of a country?s educational system.
Yet, the presentation and discussion of results presented in the aricle is too superficial. The fact that a study is defined as descriptive does not imply that it is simply sufficient to transfer numeric data in tables to written language in the body of the text. It is necessary to give substance to the descriptive analysis through the use of conceptual and theoretical elements that need to be previously worked out in the paper. It is essential to remember that theoretical elements should be considered tools for the interpretation of the data extracted from the social world.

Suggestions

The suggestions outlined below focus on the following issues: 1) context of the study 2) theoretical aspects, 3) methodological decisions and 4) presentation of results and conclusions.

1) At the level of context: The information that is presented should be elaborated and this would contribute to make the text much more comprehensible. For example, more should be said about the distinction between the types of secondary schools that is mention in the paper or about the teacher development programs to which are discussed.

(2) At a conceptual theoretical level: I think it is important to developing some concepts, especially those referred to the quality of the educational system and the opportunity structure present in society. This should be done paying particular attention to access to formal education, since these are the concepts that would develop the analysis of the data provided.

(3) At a methodological level: There are some deficiencies that can be solved by paying more attention to aspects of internal coherence of the research design. A first issue is related to sampling procedures, not explicit criteria is presented to definition of the study population or the sample size and the paper only presents the mechanisms used to select samples. As regards the type of study that is presented in the paper, the author argues that it is a study with a descriptive aim, however, the results that are presented are oriented towards ?factors which lead, to a greater or lesser extent, to the decrease in academic performance of the students?, thus in effect a causal logic prevails.

(4) At the level of the presentation and discussion of results and the conclusions of the study: Despite the descriptive goal of the paper, all effort is (discursively) aimed at proposing the existence of factors which explain the decline in the academic performance of students in recent years. From my perspective, the information on which the conclusions draw does not do allow to do this; in other words, conclusions and presented results do arise directly from the data that was collected. This question of internal coherence should be reviewed.

Final assessment

Although the main topic of the article is of permanent interest, it is also very important to address the aspects outlined above. In the current version I would not recommended the publication of the article. I would suggest a substantial revision which paid special attention to the conceptual and theoretical aspects and the issues internal coherence of the study presented above (approach goals, conceptual aspects, methodology, results, conclusions).

Police investigate Best Buy TV porn

Police are now investigating how porn infiltrated one Best Buy store's Wi-Fi network, forcing the store to issue an apology to shocked customers.

The last thing a family browsing in the TV section at its local Best Buy in Greenville, S.C., expected to see is 55-inches of pornographic images, but that's what happened earlier this week to a woman in the store shopping with her son and grandchildren. She filed a complaint with the Greenville Police Department.

And that's not the only time it happened, as the store's manager admitted it had happened the night before the woman reported her incident. An image was apparently uploaded through the Wi-Fi network and uploaded to a smart TV.

NBC affiliate WECT reported on the apology issued by the chain's corporate office:

"Two individuals accessed our store's wireless signal to broadcast inappropriate content on a smart television display. In both cases, we worked immediately to disable the inappropriate content. We greatly apologize for this unfortunate incident and we are working to ensure that it does not happen again."

UPDATE (9:37 a.m.) -- We reached out to the Greenville Police Department, and Officer Alia Urps told us that they're going through footage from in-store cameras to see who was there those evenings. Police suspect someone using a tablet or other mobile device streamed the image to the smart TV. But because there is no hard drive on the TV, it didn't ?save the offending material.
Officer Urps said that even if they were to find the culprit, "It would be difficult to prove the person sent that particular image." The charge would be disseminating obscene material.

"This is something we?ve not seen before," Urps said. "It'll be good for us to figure out what to do with this one. It'll serve as a foundation for further incidents."

Researchers Spot New Gene Mutation Linked to Breast Cancer ...

WEDNESDAY Feb. 22, 2012 -- Scientists have identified a new gene mutation linked to breast cancer.

The mutation is in the Abraxas gene, which interacts with the well-known breast cancer gene BRCA1. Abraxas organizes a large BRCA1 protein-containing complex that is needed to repair DNA damage.

The mutation affects the ability of the Abraxas protein to enter the nucleus and bind to sites adjacent to damaged DNA. This leads to genetic alterations that increase breast cancer risk, Roger Greenberg, an associate professor of cancer biology at the Perelman School of Medicine at the University of Pennsylvania, said in a university news release.

Greenberg and his colleagues screened the members of 125 families in Finland with a history of breast cancer, and found women with both the Abraxas mutation and breast cancer in four families.

The findings, published Feb. 22 in the journal Science Translational Medicine, appears to establish Abraxas as a breast cancer-susceptibility gene, according to the researchers.

It has been known that BRCA genes do not act alone in causing breast cancer, but it has proven difficult for scientists to identify other culprits. In time, this Abraxas mutation may be added to the list of 14 mutations for which women in families with a history of breast cancer can be tested, the researchers said.

The origins of Muay Thai are not clear. Muay Thai evolved from Muay Boran, an ancient form of boxing used by soldiers in conjunction with the use of weapons. Because of this, Kick Boxing has evolved even further to encompass some of the striking techniques used in boxing. Almost all techniques in MTB use the rotation of the entire body when kicking or punching. With this constant rotation of the bodys core, emphasis in training should be on developing core muscle (abdominal muscle) strength.

Striking techniques have evolved in Muay Thai from the use of simple crosses and long circular blows to techniques that incorporate more traditional forms of boxing strikes. Jabs, crosses, upper cuts, and hooks are not part of the Muay Thai repertoire. The elbow can be used as an effective striking weapon in many ways. One of the most common ways to use the elbow is to strike the opponent on the eyebrow, where the skin is close to the bone. The skin can split open, causing your opponent to bleed. The blood flow can impair your opponents vision and it can also cause your opponent to lose focus and to think about being hurt, or the potential of being hurt more. Single elbow moves are common, but a follow up elbow can be used as well. Elbows are very effective means of blocking strikes by an opponent.

There are two favorite kicks in Muay Thai, the Teh and the teep. The teep is a foot jab. The Teh is an angle kick, a rotational move using the whole body. This kick gets its power from the rotation of the body and often a counter rotation of the arms and upper body is used to increase the impact of the kick. Knees and feet are also used in Muay Thai. Learning Muay Thai and developing core muscle strength are essential parts of mixed martial arts training. ?We wouldn?t add an extra two games without reducing the preseason and we are not going to do it without the players support, so we did that in the Collective Bargaining Agreement instead of having the unilateral right, which we had,? Goodell said.? ?We determined that we were going to do this together.? We are going to make changes in the offseason and during the preseason and during the regular season to make the game safer.? If we can accomplish that we?ll look at the idea of restructuring the season and taking two preseason games away and the potential of adding regular season games, but I don?t think that will happen until at least 2013 or 14.?

Well, obviously, it won?t happen in 2012.? If the season were going to expand, plans already would have to be made for finding a way to wedge two regular-season games into a schedule that has 17 weekends of the regular season (with one bye per team) and one dormant weekend between the conference title games and the Super Bowl.? (Unless, of course, the league would be interested in starting the regular season before the weekend after Labor Day.)? And if it?s going to happen in 2013, the NFL will have to work around the February 2, 2014 date for Super Bowl XLVIII in New York/New Jersey.

Regardless, the 18-game concept remains on the table.? And even though the league no longer has the unilateral right to expand the regular season, the current CBA gives the league the unilateral right to cut the preseason in half.

As we?ve previously explained, that?s how the players would eventually be squeezed into agreeing to expand the regular season.? The league would tell the players that two preseason games are going to be dumped due to the poor quality of play and the unacceptable risk of injury in games that don?t count.? Given the current revenue-sharing model, the players then would realize that they?d be losing a ton of money if the preseason gets chopped in half ? and they?d also know (thanks to the league?s detailed studies on the matter) the extra revenue that would be generated by the addition of two regular-season games.

And so the players eventually would choose to add two regular-season games to replace the two preseason games.

Whether it happens in 2013, 2014, 2015, or beyond, the league most likely will exercise the right to dump two weeks of the preseason, and then the players will have a big decision to make.

'The Artist' dog honored at Golden Collar Awards (omg!)

The ceremony Monday at the Hyatt Regency Century Plaza Hotel proceeded like a traditional Hollywood-style awards show, with actors including "NCIS" star Pauley Perrette and "Hot in Cleveland" star Wendie Malick presenting trophies for the best dog performances on the big and small screens.

Uggie, the four-legged star of "The Artist," was named top dog in a movie. His owner-trainer, Omar Von Muller, was on hand to accept the award.

Von Muller said the Jack Russell terrier, who stole hearts in the silent film that's up for 10 Academy Awards, is a "great performer, but he's also a great family member."

"He sleeps with us," Von Muller said.

The Golden Collar Awards, dreamed up by Alan Siskind of the website dognewsdaily.com, drew other stars of "The Artist," including James Cromwell and Missi Pyle, as well as studio representatives and media outlets from around the world.

"I'm delighted to be here because I owe my career to a pig," said Cromwell, who played the farmer in "Babe."

Child abuse shrinks key brain memory centre

Maltreatment of children may stunt growth of the hippocampus, a brain region vital for memory. That's the conclusion of a study of 193 outwardly healthy adults aged 18 to 25 from the Boston area.

The stunted hippocampi could help explain how childhood stress raises the risk of psychiatric disorders in adulthood, ranging from depression, schizophrenia and post-traumatic stress disorder to personality disorders, drug addiction and even suicide.

Martin Teicher of McLean Hospital in Belmont, Massachusetts, and colleagues used standard questionnaires to reveal which volunteers had suffered abuse as children, and found size differences in regions of the hippocampus through detailed MRI brain scans.

Big differences were seen in people who said that as children they had experienced verbal, physical or sexual abuse, physical or emotional neglect, bereavement, parental separation or parental discord. Three sub-regions of the hippocampus were between 5.8 and 6.5 per cent smaller in such volunteers, compared with those who reported no maltreatment.

Stress strike

The three sub-regions ? the dentate gyrus, the cornu ammonis and the subiculum ? are all known to be vulnerable to the effects of stress hormones, which probably interfere with the formation of cells and new tissue as the immature brain develops.

"These findings support the hypothesis that exposure to early stress in humans, as in other animals, affects hippocampal development," concludes Teicher's team. They say that the study is the largest and most detailed yet to examine the phenomenon in people, and the results echo those seen in the hippocampi of rats and monkeys subjected to stress as infants.

Child abuse or poverty can also alter which genes are active in the developing brain through a process called epigenesis. These changes can lead to diseases such as schizophrenia and bipolar disorder.

"Childhood maltreatment is like a surgical strike on the brain," says Carmine Pariante, who studies the effects of stress on child development at the Institute of Psychiatry, Kings College London. "This explains why these individuals are at risk of developing a host of stress-related disorders later in life ? because they have an impaired ability to cope with stress."

"Findings like this indicate that maltreatment can leave damage hidden deep inside the body that persists for many years," says Terrie Moffitt of Duke University in Durham, North Carolina. "Once we appreciate that child maltreatment brings hidden damage that can resurface years later as memory problems, preventing child abuse seems like a very good deal."

At least your money can live like a millionaire's (AP)

WASHINGTON ? Movie super spies James Bond and Jason Bourne use them. So do real-life presidential candidate Mitt Romney, who says he pays his taxes, and untold numbers of Americans who don't. Swiss banks and their secretive counterparts around the globe may sound like the exclusive province of the wealthy, the mysterious or the shady, but anybody can legally open an offshore account.

Wouldn't it be swell to have a cool million stashed away on a sunny Caribbean island?

Here's how to do it:

Step 1: Get a million dollars.

How? There are essentially two ways ? legally or illegally. For those with dirty cash to launder ? drug traffickers, mobsters, smugglers, swindlers and such ? offshore accounts hidden from the law are the obvious choice (skip to Step 4).

For honest money, there's more to consider.

Step 2: Decide whether to tell the IRS.

U.S. citizens are supposed to pay taxes no matter where their money is. But the IRS can't tax what it doesn't know about, and the odds of getting caught offshore have been slim. But beware ? that's changing.

The government has landed some big fish ? notably the largest Swiss bank, UBS AG ? and tax cheats are getting scooped up in the net.

In an unprecedented break from Swiss legal tradition, UBS turned over the names of more than 4,000 suspected U.S. tax evaders in 2010 as part of a deal to settle conspiracy charges. Since then, the United States has been going after those people, charging more Swiss bankers with conspiracy, and leaning on Switzerland to name more names.

Other names came from a bank employee-turned-informant at the LGT Bank in Liechtenstein. And the IRS has been tracking down holders of credit cards issued from Caribbean hideaways, because the cards are a popular way to tap secret funds.

"The noose is tightening on those who want to hide money overseas," said J. Richard Harvey, a former senior adviser to the IRS commissioner.

Pressure to report Americans' holdings will increase substantially next year under a new U.S. law that imposes financial penalties on foreign banks and investment funds that don't comply. Some tax haven banks may skirt it completely, however. If they don't make any U.S. investments they can avoid the penalties.

When the IRS offered amnesty in 2009 and 2011, more than 33,000 tax dodgers came in from the cold, yielding $4.4 billion. A new round in the program opened in January.

Step 3: Look for legal ways to pare taxes.

Here it gets complicated.

For corporations, foundations, pension funds and others, controversial offshore maneuvers can help defer or avoid some taxes. For example, a corporation transfers a lucrative chunk of its business to a foreign subsidiary in a low-tax country. Or a nonprofit group puts otherwise taxable investments offshore.

"There's a thin line between tax avoidance and evasion," said Rebecca Wilkins, a senior counsel at Citizens for Tax Justice, which opposes corporate loopholes. "A lot of these transactions might not stand up in court if the IRS had the resources to pursue them."

Private equity and hedge funds flock to the Cayman Islands, which offer tax advantages for fund managers and some of their foreign and nonprofit investors. Setting up shop ? usually nothing more than a mail drop ? in these freewheeling environs also allows them to escape the tighter financial regulations of the U.S. and other nations. Some critics say that contributed to the global financial crisis.

Romney, the multimillionaire front-runner for the Republican presidential nomination, has disclosed numerous offshore investments.

His financial filings included at least six Cayman-based funds, worth between $7 million and $32 million, run by Bain Capital, the private equity powerhouse he once led. More than a dozen other funds based in the Caymans and Bermuda showed up on his 2010 federal tax returns. His campaign says he pays the same taxes he would if they were based in the United States.

Romney also had a UBS bank account in Switzerland, but it was closed in 2010 as he prepared to run for president.

Step 4: Consider other motives.

Some people want to hide wealth from spouses or business partners; doctors worry about malpractice suits; others think creditors or the government might try to seize their assets.

Wealthy residents of oppressive countries may feel safer with their savings elsewhere. Dictators, fearing revolt, often do, too.

Step 5: Choose a country.

Switzerland's famous "numbered accounts" aren't as clandestine as they're portrayed in spy movies but do cater to the rich and ensure only a few bank executives know a client's name. Hong Kong has its own version ? "chop accounts," identified by a symbol. Congressional investigators counted 50 places that can be considered tax havens or financial hideaways.

Tax havens usually boast:

_Little or no income tax;

_Laws that make it a crime for banks to reveal account holders' names;

_A history of failing to cooperate with other nations' tax collectors.

Step 6: Open an account.

Law-abiding customers who can't travel to an offshore bank can usually set up an account by mail with little or no minimum deposit.

For tax evaders and those playing the angles, a network of accountants, lawyers and bankers is ready to set up shell companies and phony trusts to hide behind.

They can get creative. Former UBS banker Bradley Birkenfeld told investigators he helped a billionaire client withdraw his funds in the form of diamonds. Birkenfeld flew to America with the diamonds hidden in his luggage, inside a tube of toothpaste.
